This post is a Community Alcohol Care Team Practitioner providing specialist bespoke, intense care and support to change‑resistant drinkers, frequent hospital attenders and more complex patients who have not engaged with other services in the community settings of Cwm Taf Morgannwg University Health Board (CTMUHB).
This is a fixed‑term / secondment for 12 months due to funding. If you are interested in applying as a secondment, please obtain permission from your current line manager prior to applying.
Main duties of the job
* Provide community nursing care to patients with alcohol misuse.
* Collaborate with mental health, clinical and nursing staff across primary and secondary care sectors to deliver highly specialised care.
* Assist the clinical team in the delivery of patient care from diagnosis to discharge.
* Provide assessment, advice, information and referral for individuals experiencing alcohol misuse problems from the general hospital setting.
Welsh language skills (desirable)
Level 3 (equivalent to CEFR B2) speaking & listening skills are desirable but not essential.
Qualifications
* UK NMC Registration.
* Clinical experience and specialist knowledge within substance misuse and gastroenterology.
* Qualification in substance misuse or therapies applicable to substance misuse.
* Registered nurse prescriber.
Experience
* Experience working within multidisciplinary settings.
Skills and Knowledge
* Ability to work unsupervised and also as a member of a team.
* Excellent oral and written communication skills.
* IT skills, knowledge of computer software and applications.
* Welsh language speaking/listening skills at Level 3 or above.
