Job Summary
As a Multi Skilled Community Care Officer, you will play a vital role in supporting adults to regain and maintain their independence. You will conduct strengths-based assessments, reviews, and support planning, focusing on achieving meaningful outcomes. Role Responsibilities
* Conduct assessments to identify individual strengths and needs.
* Develop and review support plans that promote independence.
* Identify reablement opportunities to help individuals regain skills.
* Provide short-term interventions that focus on achieving personal goals.
* Collaborate with other professionals and community services to ensure comprehensive support.
* Maintain accurate records and documentation in line with council policies.
Required Skills and Qualifications
* Have a relevant health or social care qualification (NVQ Level 3 or equivalent) or substantial relevant experience in adult social care, particularly in assessment and support planning.
* Knowledge of reablement principles and practices.
* A valid driving licence and access to a vehicle for work purposes.
