Support Worker – Discharge to Assess

Join AVS Newcastle Ltd and support individuals transitioning from hospital to home. This role is part of a regulated service requiring registration with the Scottish Social Security Council (SSSC) within 3 months of starting.


Employment Details

* Salary: £12.60 per hour
* Hours: Zero hours – as and when required, no set hours
* Contract: Zero hours, As and when required
* Location: Dundee – community based


Key Responsibilities

* Collaborate with people, families, and multidisciplinary professionals to create individualised care plans.
* Assist with activities of daily living, including bathing, dressing, meal preparation, medication management, and mobility support.
* Monitor and evaluate progress, adjusting care plans as needed to promote optimal recovery and well‑being.
* Maintain accurate documentation of interactions, care plans, and progress.
* Participate in assessments conducted in individuals’ homes with a person‑centred focus on needs, preferences, and goals.


Qualifications & Experience

* Previous experience in a similar care setting, ideally within a healthcare or social care environment (full training not essential).
* Right to work in the UK (no sponsorship available).
* Compassionate, empathetic nature with a strong desire to make a positive impact.
* Excellent communication and interpersonal skills, working effectively in a multidisciplinary team.
* Knowledge of person‑centred care principles and commitment to dignity and autonomy.
* Ability to work independently, demonstrating initiative and reliability.
* Flexibility to work various shifts, including evenings, weekends, and bank holidays.
