An exciting opportunity has arisen for an enthusiastic Health Care Assistant to join our supportive team. Havelock Grange Practice are looking for a experienced Health Care Assistant who can support the Practice Nursing Team.
The successful candidate will deliver safe, high-quality primary care services to our patients and assist the nursing and clinical team in the provision of treatment, preventative care, health promotion and patient education. Working as part of the Practice multidisciplinary team, delivering care within their scope of practice to our patients.
Main duties of the job
The Health Care Assistant will workunder the supervision of a registered clinician while developing competenciesin community-based care.
The post-holder will be an integralpart of the general practice team.
Thefollowing are the core responsibilities of the Health Care Assistant indelivering health services:
* Consultwith patients and their families to undertake assessments of patient need
* Deliver and assist clinical staff in the provision oftreatment, preventative care, health promotion, long-term condition managementand patient education
* Provideclinical support to registered nurses and other healthcare professionals withinthe organisation
* Deliverbasic clinical care interventions under supervision, supporting continuity ofcare and patient safety initiatives
* Ensurespecimens are recorded and ready for onward transportation
* Providesupport during minor operations as required
* Supportin the delivery of enhanced services and other service requirements

We also work as part of the One LifeHartlepool Primary Care Network, which has three GP Practices working inpartnership; This allows the Practice to be also supported by PCN Pharmacists,Mental Health Practitioner, First Contact Physiotherapy, Social Prescribers,Care Co-ordinators and Health & Wellbeing Coaches.
Job responsibilities
The following are the core responsibilities ofthe Healthcare Assistant in delivering health services. There may be, onoccasion, a requirement to carry out other tasks. This will be dependent uponfactors such as workload and staffing levels:
* Consultwith patients and their families to undertake assessments of patient need
* Deliver and assist clinical staff in the provision oftreatment, preventative care, health promotion, long-term condition managementand patient education
* Bean integral part of the general practice team
* Provideclinical support to registered nurses and other healthcare professionals withinthe organisation
* Deliverbasic clinical care interventions under supervision, supporting continuity ofcare and patient safety initiatives
* Ensurespecimens are recorded and ready for onward transportation
* Providesupport during minor operations as required
* Ensureclinical waste is removed from clinical areas and sharps bins replaced inaccordance with the organisations IPC Policy
* Workwithin national and local protocols where these exist
* Beaware of your duties and responsibilities regarding current legislation andadhere to practice policies and procedures on Safeguarding Adults andSafeguarding Children
* Recognisethe boundaries of their practice and know when and to whom patients should bereferred
* Supportin the delivery of enhanced services and other service requirements
* Support the administrative team on an as-required basis
* Undertakeall mandatory training and induction programmes
* Contributeto and embrace the spectrum of clinical governance
* Maintaina clean, tidy, effective working area at all times
Person Specification
Qualifications
* GCSE grade A to C in English and Maths
* Health Care Assistant Care Certificate or Qualified HCA Level 3 or above or equivalent
Experience
* Experience of working with the general public
* Experience of working in a healthcare setting
* Understanding of infection prevention and control measures
* Experience of working in a primary care environment
