Hampshire and Isle of Wight Healthcare NHS Foundation Trust - we have an exciting opportunity for you! We're looking for dedicated individuals to join our thriving Community Nursing Service across Hampshire in a permanent role, delivering outstanding care to the community in a supportive and collaborative environment.
Our Community Nursing Service provides high-quality nursing care to patients in their own homes, operating 12 hours a day (8am-8pm), 7 days a week, 365 days a year.
We pride ourselves on delivering person-centred care and ensuring our staff feel valued, supported, and encouraged to develop their skills. In this role, you'll work as part of a wider multi-disciplinary team, contributing to care planning, evaluation, and effective communication across services.
A full driving licence and access to a vehicle is required for this role. There is a space later in application to put your preferred location.
Main duties of the job
We have various locations to offer including Southampton, Hythe & Waterside, Basingstoke and Petersfield.
Please note waterside and Petersfield vacancies are part time of 3 days a week (22.5 hours).
* Work under supervision following written care plans and care pathways devised by the Registered Nurse.
* Follow prescribed care plans and treatment protocols.
* Follow HPCT Policies and Procedures.
* Has knowledge and skills to carry out prescribed tasks and will report to Registered Nurse any variances.
* Delegated responsibility for Patient Support Teams administrative tasks.
* Provide general administration support to the team.

Qualifications
* NVQ in Health care or equivalent or willingness to commit to training.
* Full UK Driving Licence.

Salary
£25,760 to £27,476 a year. Please note for part time hours the salary will be pro rata.
