Are you a physiotherapist experienced in working with adults? Are you passionate about working in a community role? Do you believe in providing the best care to every person in Leeds?

We welcome your application to join our community care bed (CCB) team across Leeds. Home first is the ideal outcome for many patients leaving hospital, but a temporary stay in a setting that is a homely environment that can provide 24-hour care is sometimes required.

The Community care beds do this, enabling early discharge from hospital and reducing hospital admissions. The CCB service delivers complex care whilst empowering patients as part of a wider multi-disciplinary team. When you join us, we will work with you to develop your own personal developmental plan that supports your learning style.

We provide training, clinical supervision, and access to a peer support network. We also offer career progression pathways to develop your clinical, leadership, and preceptorship skills. The community care beds service operates on a shift pattern and covers 7 days a week.

Based in Yeadon, you will be expected to work across all 4 hubs in the city as required. Therefore, it is essential that applicants can travel independently across the locality and have access to a suitable vehicle for business purposes. This post is 18.75 hours per week, offering some flexibility in days and times.


Main duties of the job

You will provide therapy interventions between 8am and 6pm, 7 days a week, on a rotational basis. Your role will involve working with colleagues to assess and provide physiotherapy interventions and advice as required.

You will work as part of a multidisciplinary team with registered nurses, occupational therapists, technical instructors, and support staff. You will support care home managers and staff, delivering and facilitating rehabilitation to help patients achieve their goals and manage their own care.

You will work closely with patients and their families in assessment, planning, implementation, and evaluation of treatment and care programs, using person-centred techniques to promote involvement and empowerment. Your aim is to improve recovery and increase independence by working with the team to achieve patients' goals.
