Look no further! We have a fantastic opportunity for a Social Care Officer to join our team here at Stockton-on-Tees Borough Council.
In this role you will:
1. Support the social work and care management process for individuals with functional and organic mental illness and their families.
2. Undertake assessments, reviews, and reassessments of care packages in line with departmental policies and procedures.
3. Carry out tasks delegated by qualified social workers in more complex cases, under their direction, while they retain overall care management responsibility.
4. Provide practical and administrative support to social workers in delivering high-quality care and safeguarding outcomes.
