The post holder will participate as a member of the multidisciplinary team in assessing, planning, delivering, and evaluating patients' health and well-being needs. Responsibilities include developing individual care plans, understanding the specific needs of service users and carers, and assisting in implementing appropriate actions.
You will plan and progress patient interventions with autonomous assessment and planning, teach patients and carers under supervision, promote a positive image of the organization, and be responsible for the safe use of all equipment within your scope of practice.
Coordinate complex care and therapy plans for teams to support seamless service delivery.

Are you passionate about delivering high-quality stroke rehabilitation in Lincolnshire? We seek a motivated Assistant Practitioner to support community-based stroke recovery. The service covers four bases across the county and operates a flexible, county-wide team. Applicants must be able to travel and work on a rota to cover a 7-day, 8am-6pm service.
You will support therapy staff in delivering high-quality rehabilitation to recent stroke patients. A foundation degree in healthcare (or equivalent) is essential. Experience with stroke patients is advantageous but not necessary, as training will be provided. We offer comprehensive in-house training, regular in-service sessions, and access to further learning opportunities.
