Service: Health and Social Care Partnership

Join our Children and Families Social Work team to support individuals, families, carers, and communities.

Under the guidance of a registered social worker, you'll assist with assessments, care planning, monitoring, and reviews within a multi-agency framework, ensuring services meet Social Care Standards and the needs of service users.

This role supports the Team manager and social workers in delivering high-quality services to children, families, and carers. You will contribute to team objectives, service planning, and continuous improvement while ensuring compliance with social care standards.

Key responsibilities include:

* Supporting assessments, care planning, and monitoring in child protection and multi-agency settings.
* Engaging directly with children and families, facilitating statutory contact and group work.
* Preparing reports, attending meetings, and contributing to service reviews.
* Providing advice and guidance to individuals, families, and carers.
* Maintaining accurate case records and ensuring effective communication across teams.
* Managing workload appropriately
* Supporting financial controls, ensuring resources are used efficiently.

This position requires a commitment to professional development, active participation in team meetings and training, and collaboration with internal and external partners to improve service delivery.

The post requires working 5 days over seven when necessary on a planned basis.

An ideal candidate for this role should be empathetic, resilient, and adaptable, with strong communication and teamwork skills. They should have a solid understanding of social care standards and legislation, along with excellent organisational and problem-solving abilities. A commitment to professional development and a genuine desire to support children and families in achieving positive outcomes is essential.
