Role Overview
Within Locala’s Integrated Communities, we deliver multidisciplinary community services, including health and social care in collaboration with Kirklees Council. We are seeking a motivated and enthusiastic Staff Nurse to join our Intermediate Care Team.
Key Responsibilities
* Support medical and surgical service users from hospital discharge to their journey home.
* Provide diverse care, including complex wound care and monitoring deteriorating patients.
* Work collaboratively within a multidisciplinary team to rehabilitate service users, prevent hospital admissions, and promote safety.
* Participate in a 7-day service, working 8 am to 7 pm in a rota pattern, assessing, treating, and evaluating clients' needs holistically and patient-centered.
Team and Environment
Our Nursing Team offers a supportive environment for professional development, working alongside GPs, Physician Associates, Occupational Therapists, Physiotherapists, Pharmacists, Healthcare Support Workers, and the Kirklees Care Team. Our goal is to enable clients to return home safely and avoid unnecessary hospital or long-term care admissions.
