Responsibilities
* Provide advice and patient management relating to complex discharge scenarios.
* Work in a collaborative manner with the multidisciplinary team to facilitate appropriate and timely discharges by providing continuity in discharge planning and effective dissemination of information.
* Ensure all relevant social service referrals are made in a timely manner, monitor response times, act on delays and work with the ward team to ensure smooth patient flow regarding discharge.
* Act as a point of contact for the ward regarding discharge and ensure the ward carries out the responsibilities outlined in this Job Description.
* Be proactive with regards to patient flow and discharge.
* Attend MDT Board Rounds, collate detailed information about each patient, escalate diagnostics and specialty reviews, arrange patient transport bookings, communicate with patients and families, liaise with pharmacy, ensure a clear MDT plan, and provide effective written documentation about discharge progress.
* Operate in a computer-literate environment with excellent written and verbal communication skills.
Role and Team
The Assistant Patient Flow Co-Ordinator role is a varied role with a primary focus on patient flow and patient discharge. You will be the point of contact on the ward for information and advice to staff, patients, and relatives/visitors. You will work collaboratively with the Multi-Disciplinary Team (MDT) to facilitate safe and timely discharges by providing continuity in discharge planning and effective dissemination of information.
Requirements
* Strong communication skills, both written and verbal, and computer literacy.
* Ability to work as part of a team and manage multiple discharge-related tasks.
Contract and Working Arrangements
***Please note this post is a 6 month Fixed Term Contract and we are unable to sponsor the role***
This will be a full-time post working Monday to Friday shifts covering 08:00-16:00 and 10:00-18:00. You will be employed under the Flow Hub at Broomfield and will work within a team covering a group of wards within a zone. You may be required to cross cover as the service requires.
