How You Will Contribute
* Coordinate with the assigned nurse to provide safe patient care and report on the patient’s condition.
* Perform personal care, take vitals, communicate with families, and other duties as assigned.
* Travel between patients and work as part of an interdisciplinary team supporting families and patients during their end-of-life journey.

Qualifications
* Meet state training requirements
* Minimum six months experience providing personal care
* At least 18 years old
* Complete 75 hours of classroom and supervised practical training (16 hours prior to practical training)
* Current CPR certification
* Ability to read and follow written instructions and document care
* Understanding of hospice philosophy and comfort with caring for the terminally ill
* Ability to work with minimal supervision
* Ability to handle death/dying
* Strong communication and organizational skills
