Hours: Full time (35 hours per week)

Location: Sunderland, Tyne and Wear

Salary: £25,933 per annum (Fixed Salary)

Job Title: Essence Dementia Navigator (Phase 1)

Position Overview: Provide one-to-one support for people with mild to moderate dementia and their carers through tailored personal plans, enabling them to live fully and actively in the community.


Key Responsibilities

* Develop and implement individual personal plans for service users.
* Deliver one-to-one support and care to dementia patients and their carers.
* Communicate effectively, both verbally and in writing, with service users, carers, and team members.
* Use information technology skills to record and manage user information.
* Work collaboratively within the Essence Service team.


Required Qualifications and Skills

* Experience and knowledge of working with individuals with dementia.
* Excellent verbal and written communication abilities.
* Strong IT skills.
* Full clean driving licence and access to a private vehicle.
* Will be subject to a DBS Enhanced Disclosure check.
