East Dunbartonshire Council Salary : Grade 16 Salary: £108,453.81 - £116501.63 per annum (pro rata for part time)
Tenure: Permanent
Hours: 37 hours per week
Location: Kirkintilloch Health and Care Centre

East Dunbartonshire HSCP is seeking an exceptional, values-driven systems leader to take on the pivotal role of Head of Interface Health and Care Services. This is a rare opportunity to shape and deliver integrated health, social work and social care services that directly impact the lives of individuals, families and communities.

With responsibility for a diverse and complex portfolio, you will lead the strategic planning, operational delivery, performance management and continuous improvement of interface services across East Dunbartonshire. Your work will ensure that people are supported to live independently, safely and meaningfully in their own homes or homely settings for as long as possible.

The Service

The Interface Health and Social Care Services business unit of East Dunbartonshire HSCP has a combined budget of c. £92m and c.540 headcount of staff. The business unit comprises of the following service groups:

* Interface Social Work Services (Locality, Hospital Assessment, Care Homes)
* Interface Health Services (Adult Community Nursing Services, Hospital at Home and Community Rehabilitation)
* Independent Living Services (Care at Home, Telecare, Occupational Therapy, Sensory Impairment, Equipment and Adaptations)
* Empowering People and Communities (Self Directed Support, Local Area Co-ordination for Older People, Older People Social Support (inc day care), Operational Health Improvement/Community-Led Support)
* Urgent and Unscheduled Care (including prevention of avoidable admission and delayed discharges)

The business unit also leads primary care independent contractor support and development, and holds significant spend in commissioned service provision.


The Role

The Role

Reporting directly to the HSCP Chief Officer as part of the HSCP Senior Management Team, you will:

* Provide strategic and operational leadership across a wide-ranging portfolio including:
o District and specialist nursing services
o Care at Home services and residential and nursing care commissioning – including direct management of internal registered care services
o Community rehabilitation and Hospital at Home services
o Primary care transformation and prescribing
o Locality and interface social work services Urgent and unscheduled care strategic portfolio
* Ensure high-quality, seamless service delivery across health and social care interfaces, with a strong focus on prevention, early intervention and reducing avoidable admissions.
* Act as the key strategic link for interface services within NHS Greater Glasgow and Clyde and local partnerships.
* Drive innovation and continuous improvement, drawing on best practice locally, nationally and internationally.
* Ensure compliance with the Health & Care (Staffing) (Scotland) Act 2019, including across commissioned services.
* Lead on population health and wellbeing and participate in community planning
* Act as Deputy Chief Officer for the HSCP

* A Master’s degree (or equivalent) and a relevant professional qualification in health, social care, social work, community development or management
* Strong experience in health and/or social care leadership

* Experience of political management and strategic influencing
* Above all, you will demonstrate integrity, resilience and a passion for improving outcomes for people and communities.
