What you will do:
Serve as a key point of contact and support for young people leaving care.
Coordinate support and resources from various agencies to ensure smooth transitions into adulthood.
Provide direct work and targeted interventions for young people aged 16 to 25, helping them secure education, employment, and stable housing.
Engage with young people, especially those harder to reach, fostering connections and enhancing life outcomes.
Support and challenge families and agencies to drive improvement in outcomes for care leavers.
Deliver a duty service to provide crisis support and ensure ongoing advice and guidance are available.
