Job overview

Main duties of the job

To provide responsive, individualized information, support and signposting service to people with memory loss and an early diagnosis of dementia, who are not in secondary care services, or subject to care coordination as part of the Mental Health Measure. Developing and maintaining excellent communications with primary care in the community network clusters, at the interface between primary care, integrated community services and secondary care services, the post holder will offer high quality information advice and support to people their families and carers affected by the early stages of memory loss and dementia. Responding to referrals from primary care professionals or members of the integrated community network teams, the community resource team, and professionals within Older Peoples Mental Health Services

Essential Qualifications & Knowledge

Essential criteria

1. A minimum of 4 GCSEs (Grade A – C) which must include English language or the Welsh equivalent
2. Quality Credit Framework qualification at level 3 e.g. NVQ or OCN related to Health & Social Care and/or equivalent experience
3. Documented evidence of continuous development and participation in training activities both internal and external relevant to working with people affected by dementia.
4. Knowledge of dementia and how this may impact on the physical, psychological and social aspect of the person’s life as well is their family and carers, and be able to demonstrate how to support people living with these issues

Desirable criteria

5. Developing knowledge of how dementia affects people’s daily lives and have experience of working with a range of statutory and voluntary agencies linked to mental health/dementia services

Essential Experience

6. Experience of working with individuals or in groups of people affected by memory loss or with a diagnosis of dementia
7. Have experience of working in inter-agency or multidisciplinary team settings
8. General health, safety and risk awareness, with the ability to relate this to individual circumstances, analyse actual risk and take appropriate actions

Desirable criteria

9. Experience of working in the community or in mental health care settings

11. Good communication and interpersonal literacy and numeracy skills with effective verbal communication skills with people from diverse backgrounds
12. The ability to work with people and support them to manage change, demonstrate self-motivation and possess effective organisational skills
13. Ability to travel across a determined area working with a number of people (preferably a car owner/driver in possession of a full UK driving licence) and giving high quality information advice and carry out relevant support tasks
14. Computer skills e.g. use of Microsoft 365 (Teams, Word, Excel, Powerpoint etc) and Data inputting
15. Ability to work as part of a team and awareness of team dynamics.
16. Effective written communication skills and ability to keep accurate and legible documentation
17. Ability to work collaboratively at the interface between primary and secondary third sector services
18. Demonstrate a motivated, committed and enthusiastic approach, with a commitment to maximising the quality of advice, care and support delivered
19. Demonstrate enthusiasm and commitment to meet the needs of people living with the effects of dementia as well as their family and carers
20. Flexible and adaptable and be self-motivated.
21. Good time management and ability to use initiative appropriately
22. Ability to promote a professional image of the organisation

23. Welsh Language Skills are desirable levels 1 to 5 in understanding, speaking, reading, and writing in Welsh
