Main duties of the job
* Lead proactive, evidence-based nursing care for people living with long-term conditions, supporting self-management and preventing avoidable deterioration.
* Provide high-quality treatment room and preventative care services across the age range, guided by current best practice and professional standards.
* Offer clear health education and personalised advice, helping patients understand their conditions and treatment plans.
* Work closely with GPs, HCAs, pharmacists and wider MDT colleagues to coordinate safe, joined-up care.
* Use clinical systems to record accurately, monitor outcomes and contribute to quality improvement, safety audits and practice targets.
* Play an active role in safeguarding, infection prevention and maintaining a positive, patient-centred culture at the surgery.

* Deliver care for patients with chronic diseases, including asthma, COPD, diabetes, and hypertension
* Provide cervical cytology (smears) in accordance with national screening guidelines
* Administer vaccinations and immunisations (childhood, adult, and travel)
* Perform wound care, dressings, and removal of sutures/staples
* Assist with family planning, contraceptive advice, and NHS health checks
* Undertake ECGs, blood pressure checks, and other routine procedures
* Support infection prevention and control within the practice

* Registered General Nurse (RGN) with current NMC registration
* Completion of a Practice Nurse Induction Programme or General Practice Nurse (GPN) Foundation Course
* Experience in a general practice or primary care setting
* Competent in chronic disease management, cervical cytology, immunisations, and wound care
* Excellent communication, record-keeping, and time management skills

* Additional training in travel health, smoking cessation, contraception, or spirometry
