Key Responsibilities Carry out the full Occupational Therapy process with service users and carers, promoting independence and choice.
Assess needs and risks in partnership with service users and carers to ensure effective, person-centred support.
Conduct risk assessments related to moving and handling tasks.
Advise and assist service users in selecting appropriate aids and adaptations in line with relevant legislation, including the Children and Families Acts 1989 and 2004 and the Care Act 2014 .
Collaborate closely with Social Care, Health, Housing, and voluntary sector professionals to deliver holistic care.
Participate in, and occasionally lead, multidisciplinary assessments and case conferences.
Support service users in developing their own support plans through creative, person-centred approaches.
Requirements HCPC registration is essential.
A full UK driving licence is required, as community visits form part of this role.
