We are looking for anexperienced Advanced Nurse Practitioner to join our enthusiastic practice team working 4 or 5 days a week. Ourpractice is based in the Primary Care Centre in Hexham serving 10,000 patients.
The successful applicantwill work closely with our existing Advanced Nurse Practitioners, GPs, Paramedic, Practice Nursing team and reception team todeliver an urgent care service to our patients of all ages. Your working daywill be varied, delivering a wide range of evidence based care for patientswith acute illness and minor injury.
The Nurse Practitioner will work autonomously, albeit with the support of colleagues. They will consult with patients of all ages and will assesses, diagnose and treat patients with a range of acute, non acute and chronic medical conditions.
The Nurse Practitioner works autonomously and is accountable for their own professional actions. They will:
Undertake Clinical Nursing Practice using expert knowledge and clinical skills to deliver holistic care to people accessing primary health care services.
Assesses, diagnose and treat patients with a range of acute, non acute and chronic medical conditions.
Assesses, diagnose and treat patients in the surgery, their own home or using remote care (eg telephone, or video consultation).
Arrange the direct admission of patients to secondary care who are in acute medical need.
Refer patients to other medical specialities for assessment if deemed appropriate.
Refer patients as appropriate to other members of the multidisciplinary team, secondary care and to other statutory and voluntary organisations.
Actively implement safeguarding protocols for children and vulnerable adults and to know who to refer to and how to escape concerns about radicalisation.
Interpret a range of diagnostic tests and undertake routine clinical procedures.
Work within professional guidelines and codes of conduct.
Work within all relevant practice policies and procedural guidelines e.g. infection control, chaperoning, risk management.
Provide clinical input where required alongside Practice Nurses and Healthcare Assistants, particularly newly appointed nurses and GPs, to assist in skill development and sharing of best practice.
