Job overviewThe Renal Team Leader Dietitian will be responsible for providing high level clinical dietetic leadership of the Renal Dietetic service provided by University Hospitals Plymouth NHS Trust (UHP).The post-holder will deliver specialist clinical care as part of a multi-professional team, acting as the lead source of knowledge and expertise within the renal dietetic team at UHP.The post holder and renal team provide a dietetic service to people living with advanced kidney disease across Plymouth, South Devon and East Cornwall including pre-dialysis, haemodialysis, peritoneal dialysis, transplants, conservatively managed and general medical patients under the care of the renal physicians. To undertake a team leader role with support from the Head of Nutrition and Dietetics in planning, co-ordinating, delivering, developing and evaluating the dietetic care to a wide range of patients within the renal team’s remit.To work as a senior autonomous Nutrition and Dietetics practitioner, with responsibility for a diverse and complex individual caseload of adults.To provide clinical leadership to more junior staff in the Department and ensure governance and quality agenda is delivered in line with trust structure.The post holder is involved in all Nutrition and Dietetic departmental activities and contributes to the delivery of a Trust-wide Dietetic service.Main duties of the jobUndertake a team leader role, with support from the Head of Nutrition and Dietetics in planning, co-ordinating, delivering, developing and evaluating the Dietetic service provided to renal patients, on a day-to-day basis.Manages on a day-to-day basis the renal dietitians and support staff ensuring the service is run efficiently and clinical capacity is optimised, as directed by the Head of Nutrition & Dietetics.To provide cross-cover managerial supervision to other clinical teams as directed by the Head of Nutrition and DieteticsTo take a major role in the advanced assessment and treatment of patients within the renal speciality who may have acute complex and/or acute on chronic presentation, and to determine clinical diagnosis and dietetic treatment indicated, and to maintain records as an autonomous practitioner.Undertakes evidence-based audit and quality improvement / research projects to further their autonomous practice and the clinical practice of the team. Make recommendations to clinical lead/manager of service for changes to practice by the team.

The post is based at Derriford Hospital, which includes the Plymouth Dialysis Unit. The post holder and renal team provide a dietetic service to daycase and evening patients at main centre and satellite dialysis units and to out-patients at clinics across the UHP renal geographical area. Responsible for the professional organisation, delivery and management of the dietetic service relating to the renal dietetic team2. To undertake the appropriate assessment and treatment of patients referred for dietetic advice as necessary and ensure that correct medical ethics are always observed.3. To manage own clinical caseload of patients referred to the service as well as renal patients in both inpatient and out-patient setting.4.

To lead on ensuring that patient nutrition outcomes are clearly completed and documented for the dietetic service, as directed by the Head of Nutrition & Dietetics5. Liaise with the stakeholders of other organisations concerning the provision of dietetic services relating to the renal dietetic service provision.6. To support Head of Nutrition & Dietetics in providing excellent clinical leadership across the renal dietetic service.7.

To set a high personal standard of work and facilitate other staff to do likewise.8. To liaise with staff of other disciplines, as and when necessary, to ensure that effective communication and reporting always takes place.9. Ensure continuing professional development of all staff by identifying training needs and in collaboration with others developing and implementing in-service and post registration training programmes.10.

Be responsible for the collection of statistical data relevant to the service in line with Trust and departmental policies and procedures.11. To manage own clinical caseload of patients referred to the out-patient service and wards. To determine own work priorities, balancing clinical, professional and organisational demands.12.

To provide an advisory service on renal dietetics service provision to other health service personnel and advise on and participate in staff training programmes.13. To maintain an up-to-date knowledge of developments in renal dietetics and an awareness of best practice by liaising with colleagues, reviewing literature and attending appropriate meetings and courses.14. To provide training and supervision to Nutrition and Dietetic students on clinical placements in line with the University’s syllabus and student assessment procedures.15.

Represent the profession at clinical working groups, steering groups and primary care service planning.16. To be professionally and legally responsible and accountable for all aspects of own work, including the management of patients in own care.17. Communicate with patients, carers who may be distressed and / or may be terminally ill.18.

To support, advise and mentor new and less experienced team members both formally and informally.19.
