Occupational Therapist at Manchester University NHS Foundation Trust (MFT). Closing date: 24 September 2025
The post involves working within the discharge to assess team delivering holistic assessments & interventions to patients in their own homes/ place of residence. Independent travel within the local geographical area to deliver the role is essential.
The Manchester community response services offer a seven day service:
Discharge to Assess - delivered over 7 days 08:00-20:00
With the above in mind, you must have experience of working in the NHS and in community services. The successful candidate will be able to demonstrate an appropriate level of clinical skills in occupational therapy, will be experienced in administering person-centred assessments, as well as implementing and evaluating occupational therapy interventions in a fast paced working environment. You must have a recognised Occupational Therapy qualification and current HCPC registration.
Main duties
* Manage a clinical caseload of patients, including those with complex and highly complex needs.
* Supervise band 3 support staff with caseload management.
* Work without direct supervision as a lone worker in the community assessing, planning, implementing, evaluating and modifying individualised treatment programmes in patients' homes for patients with low to highly complex needs.
* Work as a team member, collaborating with colleagues to ensure patient needs and goals are met using approaches which promote choice, participation and quality of life.
* Ensure patients are triaged and managed on appropriate and timely pathways of care by liaising closely and working in partnership with other health and social care professionals including GPs, consultants, specialist equipment agencies, social service partners and voluntary agencies.
* Support patient flow by contributing to the timely discharge of patients from the acute setting.
* Provide complex professional and clinical advice to patients, family members, carers and health and social care professionals including GPs, therapy colleagues and care managers.

Person Specification
Qualifications
* Honours degree in Occupational Therapy / Occupational Therapy Diploma
* Evidence of continuing professional development including relevant attendance at postgraduate courses and conferences within the clinical speciality
* Registration with Health Professions Council
* Further health related qualification
* Membership of relevant special interest groups
Experience
* Significant band 5 experience
* Evidence of experience with a broad range of general rehabilitation conditions
* Previous experience at band 6 level
* Previous community experience
Skills and Knowledge
* Ability to manage complex caseload
* Ability to work autonomously and as part of an MDT
* Well developed communication skills with the ability to communicate and extract complex information
* Ability to produce clear concise written information
* Ability to demonstrate a sound understanding of clinical governance and risk
* Knowledge of research and audit
* Experience of supervision of occupational therapists or rehabilitation assistants
