Our Community Teams provide compassionate, person-centred care to people living with progressive and life-limiting illnesses across Stoke-on-Trent & North Staffordshire. Working alongside Clinical Nurse Specialists (CNS) to deliver holistic, responsive, and person-centred care to patients in the community.
As a Senior Health Care Support Worker, you’ll play a key role in coordinating day-to-day patient contact within a community geographical team. This role requires excellent communication skills, confidence working independently, and a caring, organised approach to supporting patients, families and colleagues.
You’ll contribute to telephone support, home visits for non-complex patients, and the smooth running of the community service. This role is ideal for an experienced Health Care Support Worker who is passionate about palliative care and is ready for a new role, new challenges and to progress within a supportive, values-led organisation.
Key Responsibilities
* Support the coordination of patient contact and workflow using clinical systems
* Manage and monitor team emails, tasks and telephone activity
* Provide non-complex telephone support to patients and families, including bereavement calls
* Undertake non-complex home visits and joint visits alongside Clinical Nurse Specialists
* Observe, report and escalate changes in patient condition appropriately
* Liaise with GPs, District Nurses and other external professionals
* Support referrals and audits to enable holistic patient care
* Maintain accurate, timely and confidential patient records
* Work collaboratively as part of a multidisciplinary community team
* Attend mandatory training and palliative care education
About You
We’re looking for someone who is compassionate, reliable and confident working in the community, with experience supporting patients and families during challenging times.
You’ll bring:
* NVQ/QCF Level 2 in Health & Social Care (or equivalent)
* Substantial experience as a Health Care Support Worker
* Experience working with patients at end of life or with complex health needs
* Confidence working with indirect supervision in a community setting
* Strong communication and organisational skills
* Ability to observe change, escalate concerns and work within care plans
* IT skills and experience maintaining electronic records
* A full driving licence and access to a suitable vehicle
A Level 3 qualification, palliative care training or telephone review experience would be an advantage, but we’ll support development where needed.

Salary: £24,100 per annum
Hours: 37.5 hours (part time considered)
