Overview

*PREVIOUS APPLICANTS NEED NOT APPLY* A fantastic opportunity has arisen for a pharmacist to join the medicine team at the West Middlesex site of Chelsea and Westminster NHS Foundation Trust. We provide clinical pharmacy services to the Acute Assessment Unit, Ambulatory Care and Emergency departments, as well as a range of specialties including Care of the Elderly, Endocrinology, Respiratory, Gastroenterology. The post-holder will rotate between different specialties, taking responsibility for the provision of clinical services to a specialist team. This role provides an excellent opportunity to develop clinical, managerial and leadership skills.

Responsibilities

* To provide a specialist clinical pharmacy service to patients within the Emergency and Integrated Care Division (EIC).
* To ensure the safe, effective, legal and accurate use of medicines within the specialty and to support and supervise less experienced pharmacists on rotation.
* To act as a role model and to deliver, develop and evaluate specialist clinical pharmacy services in accordance with the objectives set by the line manager.
* General Clinical Practice – liaise and work closely with the senior clinical pharmacy team to develop clinical pharmacy services to the Division including ward pharmacy and medicines management.
* Aim to provide safe, clinically effective and cost efficient use of drugs in the patients as allocated.
* Make appropriate referrals; ensure clear and effective communication of patients’ pharmaceutical needs to other pharmacists and health care professionals both within the trust and in other health care environments.
* Demonstrate and apply general pharmaceutical knowledge, and where applicable, specialist clinical pharmacy knowledge to plan, manage, monitor and review therapeutic programmes for patients allocated, ensuring quality of patient care.
* Participate in ward rounds to integrate into the multidisciplinary team and raise the pharmacy profile via contributions at consultant level.
* Demonstrate appropriate clinical reasoning and judgement. Work across traditional boundaries, including the transcription of discharge medication.
* Be readily available to answer any general medicine related pharmaceutical queries from patients, pharmacists and other staff within pharmacy and from other health care professionals including medical staff.
* Proactively provide advice and information on pharmaceutical issues within the directorate to pharmacy staff and other healthcare professionals as needed. Demonstrate awareness and commitment to the Trust’s Clinical Governance Agenda.
* Support the maintenance and development of Medicines Management Initiatives including use of patients own medicines and self-medication. Facilitate writing of discharge prescriptions and develop accreditation schemes for this purpose.


Leadership

* Act as a role model for junior pharmacists, trainee pharmacists and pharmacy students on placement in the rotation.
* Promote best practice e.g. evidence-based practice through prescription monitoring.
* Implement and/or follow agreed clinical governance initiatives such as incident/error reporting and investigation.
* Develop clinical pharmacy services in line with local and national objectives and contribute to pharmacy research initiatives within the department.
* Be involved in the process of recruitment and selection of staff.


Management of service

* Ensure national and local agendas are delivered. Follow and conform to relevant standards of care. Identify and manage risks in relation to medicines use according to policy/protocol.
* Delegate duties to junior staff. Evaluate the performance of junior staff using departmental/Trust IPR tools and General Level Competence Framework.
* Provide evaluated pharmaceutical and financial information for the directorate. Identify changes in prescribing practices that may impact the drugs budget and report drug usage and expenditure to the Division.
* Deputize for the Advanced Pharmacist AMU/AEC and other senior clinical pharmacists as required; represent pharmacy at acute medicine specialty meetings.


Evaluation of Service

* Monitor clinical pharmacy to ensure safe, clinically effective and cost efficient use of drugs in patients.
* Document workload and quality of pharmacy services in line with objectives. Perform audits of drug usage within the Directorate as part of the multi-disciplinary audit process (e.g., NICE, CLAHRC research, local priorities).
* Undertake assessments/evaluations as required (e.g., new products within the allocated speciality).


Research and Service Development

* Demonstrate a critical approach towards pharmacy practice. Integrate research evidence into practice.
* Develop clinical pathways within the specialty area. Seek to improve ward and clinical services. Participate in pharmacy practice research in line with CLAHRC agenda and relevant studies.
* Assist with supervision of undergraduate and postgraduate projects. Support continuous improvement within Pharmacy.


Education and Training

* Participate in training and supervision of junior pharmacists, trainee pharmacists and pharmacy students. Identify training needs and develop strategies to meet them.
* Provide education and training to pharmacy and other staff. Evaluate training; participate in clinical training of medical and nursing staff.
* Maintain a portfolio of CPD and participate in Departmental Education and Training Programme. Act as a GPhC trainee pharmacist Practice Supervisor/Designated Supervisor as appropriate.
* Participate in development of current and future pharmacy workforce; co-ordinate department meetings and teaching sessions.


Other

* Provide dispensary cover to inpatient and outpatient pharmacy per rota. Be responsible for clinical pharmacist oversight of Patient Services in the absence of the Dispensing Services manager.
* Ensure accurate screening and final checking of medicines including oral chemotherapy, HIV and clinical trials; ensure formulary and prescribing policy adherence.
* Maintain broad pharmacy practice knowledge; ensure communications are clear, appropriate and inclusive; consider language and cultural barriers.
* Participate in education and training programmes; comply with relevant legislation and Standards for Pharmacy Professionals (GPhC).
* Participate in on-call, weekend/bank holiday and shift working as required and in Practice Research.
