About The Role As a Case Manager with ILS, you’ll play a vital role in supporting young people and adults following life-changing injuries. About You The Skills You'll be Sharing With The Team You will be an experienced health and social care professional who has extensive expertise of working with young people and adults with catastrophic injuries; including traumatic brain injury, poly-trauma, spinal cord injury, limb loss and cerebral palsy. You will have experience of working directly with clients in the community and be able to demonstrate your professional abilities, within the field of complex, long-term health care assessment, care planning and achieving positive outcomes for clients. You are able to work autonomously and in a proactive, solution focused way using sound clinical reasoning in your actions. You must have current HCPC/NMC/Social Work England registration and an up to date CPD portfolio.
