The following is essential criteria: Degree in Social Work SWE Registration Right to work within the UK Bolton are looking for a Social Worker for the Neighbourhood team, any adult SW 18+ with any need other than Learning Disability. The candidate will focus on outdated reviews, approximately 4 per week. Candidates with experience in ongoing reviews from other authorities are ideal. The candidate will work with health professionals such as district nurses, therapists, etc. Required experience and knowledge: Care Act assessment Strength-based support planning BIA and capacity reviews Safeguarding Candidates must have at least 2 years of experience within a specific neighbourhood team; otherwise, 3+ years are required. Candidates should be local to Bolton and able to work in Bolton, with hybrid working arrangements. The role is based in Bolton, Bury, Wigan, and requires flexibility to attend the office or complete visits on short notice. The role involves a 50/50 split between office and hybrid working, with full office presence required for the first 4 weeks. This is an exciting opportunity to join Bolton’s Adult Central South Neighbourhood team. You will work closely with health colleagues to support adults and their carers, assessing and providing care services under the Care Act 2014. Bolton's teams include Social Workers/Assessors, Occupational Therapists, Nurses, and Mental Health Practitioners, focusing on person-centered and strength-based practices to deliver high-quality health and social care, protect vulnerable individuals, and foster supportive communities. The role includes undertaking assessments, providing ongoing support, working with complex issues, family dynamics, legal matters, crises, and transferring cases as needed. Main duties involve completing Care Act assessments, devising care and support plans, reviews, Mental Capacity Assessments, participating in Best Interests meetings, and safeguarding investigations. Utilizing resources from social care, health, voluntary, and statutory agencies, the role involves assessing independence and ongoing support needs, administrative functions, attending case conferences, reviews, and representing the Council in court when appropriate.
