The Role As a Social Care Practitioner, you will be responsible for: Working collaboratively with individuals to assess their social care needs and provide appropriate support and interventions. Conducting assessments and reviews of care packages, ensuring that individuals receive the necessary support to maintain their independence and well-being. Building positive relationships with service users, their families, and other professionals to ensure holistic care delivery. Providing information, advice, and guidance to individuals regarding available services and support options. Advocating for the rights and needs of individuals, promoting their independence and inclusion within the community. Working in partnership with external agencies and providers to coordinate care and support services. Contributing to the continuous improvement of services through participation in team meetings, supervision, and professional development activities. The ideal candidate will possess the following skills and attributes: A relevant social care qualification Experience working in a social care environment, ideally within an adult social care setting. Sound knowledge of social care legislation, policies, and best practices. Strong assessment and care planning skills, with the ability to identify and address individual needs. Excellent communication and interpersonal skills, with the ability to build rapport with service users and collaborate with multidisciplinary teams. Empathy, compassion, and a person-centred approach to care delivery. Ability to work independently, manage a caseload effectively, and prioritise tasks. Knowledge of safeguarding principles and the ability to identify and respond to risks.
