Job Title: Social Care Worker (Band 4)
Location: Wade Centre, Kingussie
Responsibilities
* Act as a key worker for care home service users by initiating, developing and reviewing the care plan for those individuals.
* Work in a person‑centred way which promotes the quality of life and maximises independence for all service users.
* Support the reablement of service users, including short‑term as well as long‑term residents.
* Act as shift leader and take charge of the Care Home as necessary.
* Act as key worker for a number of service users, who may be resident in the Care Home or who may use day‑care facilities. This may include continuing to work with these service users if they are temporarily moved to other facilities, for example for recuperation or reablement.
* Follow appropriate training to act as shift leader as and when required, being responsible for all staffing matters during that period. Support reablement plans for service users, administer medication as prescribed, organise and support social activities and outings, and enable service users to maintain contacts in potentially isolating situations.
* Provide support to service users at times of loss. Ensure personal care tasks are implemented, either through own intervention or by Social Care Assistants, including ensuring service users are comfortable, can eat and drink, can access and use toilets, are assisted with continence problems and can maintain personal hygiene and appearance. Maintaining dignity of service users is paramount.
* Act as a key worker in relation to infection control and ensure best practice in moving and handling service users. Contribute to the care of a deceased person, including performing last offices if required, and contribute to the planning, monitoring and recording of service delivery.
Works under the general guidance of the Care Home manager, the Deputy Care Home manager or the shift leader, but uses own experience, training and judgement when working with service users. Allocates tasks to other Social Care Workers and Social Care Assistants when acting as shift leader. Makes decisions about overall welfare and safety of service users when in charge of Care Home and acting as shift leader. Assesses progress of service users against care plan and adjusts accordingly, taking into account the service user’s interests and priorities as well as any changes in condition.
