To provide high quality patient care in the community setting under the direction and supervision of a senior nurse/practitioner. To support the assessment of the health and health related needs of patients, clients, their families and other carers. To assess, plan, provide and evaluate nursing care to meet care needs of individual patients and carers in their own homes.
