Graceful Home Care Ltd is seeking compassionate and reliable Domiciliary Support Workers to provide high-quality care and support to individuals within their own homes across Wales.
This role involves delivering person-centred support in accordance with individual care and support plans, promoting independence, dignity, choice, and wellbeing in line with the Regulation and Inspection of Social Care (Wales) Act 2016 and CIW standards.
The successful candidate will support individuals to remain safely in their own homes while maintaining their rights, privacy, and personal preferences.
Key Responsibilities
* Deliver person-centred care in accordance with individual Personal Plans
* Support individuals with personal care, including washing, dressing, toileting, and grooming
* Assist with mobility and safe transfers using appropriate moving and handling techniques
* Support with medication administration in line with company policy and MAR documentation
* Provide support to individuals living with dementia, physical disabilities, or complex needs
* Prepare meals and assist with nutritional support as required
* Carry out light domestic tasks to maintain a safe home environment
* Provide companionship and emotional support to promote wellbeing
* Maintain accurate, clear and timely care records
* Safeguard individuals by recognising and reporting concerns appropriately
* Follow infection prevention and control procedures
* Work in partnership with families, healthcare professionals, and the wider care team
Essential Requirements
* Commitment to person-centred care and the wellbeing of individuals
* Good communication and record-keeping skills
* Ability to work independently within the community
* Willingness to complete mandatory training and continuous professional development
* Registration with Social Care Wales (or willingness to register within required timescales)
* Right to work in the UK
Desirable
* QCF Level 2 or 3 in Health and Social Care (Wales)
* Experience in domiciliary care or community-based support
* Valid UK driving licence and access to a vehicle
* First Aid and Medication Administration training
