We are delighted to open applications for a dedicated Patient Coordinator (RVN) to join our team at Hamilton Specialist Referrals on a 12-month fixed-term contract basis. Our successful candidate will work collaboratively within our clinical nursing team and act as a point of contact for our clients.
Coordinating a seamless client experience
Join our nursing team at Hamilton Specialist Referrals as a Patient Coordinator, where you'll play a crucial role in delivering exceptional client service and seamless patient care coordination. From admission to discharge, you'll be the central liaison, ensuring clear, compassionate communication with clients and working closely with clinicians and the Clinical Administration team. Clinically, you will be based predominantly within our Wards team, providing high standards of care to a range of complex referral cases.
Key responsibilities include, but are not limited to:
1. Client Communication: Provide a consistent and client-focused experience by keeping clients informed throughout their pet's journey, addressing questions, and providing guidance on care plans.
2. Patient Care: Oversee patient admissions and discharges, manage inpatient care coordination, and work clinically as part of our wards team. This includes I/V catheter placement, blood sampling, TPR & BP assessment, sedation/recoveries, and nurse consults.
3. Team Collaboration: Act as a pivotal link between clients, clinicians, and the Clinical Administration team to maintain clear and effective communication, supporting the best possible patient outcomes.
Supporting you in your new role
* Experience within a referral setting is preferred but not essential.
* We offer central support via IVC Evidensia's 'Introduction to Referral Nursing' training modules, designed for RVNs transitioning from first opinion to referral practice.
* You will be a proactive, enthusiastic, and approachable individual who is a great team player, able to use initiative and self-motivate.
* Excellent communication and interpersonal skills, with experience in client liaison.
* You must be a qualified RVN with current RCVS registration.

Working Pattern
This is a part-time position, 15 hours per week, from 16:30 to 19:30, Monday to Friday. There is a maximum of 2 bank holiday shifts per year, for which TOIL is provided. Please note, this is a maternity cover position.
