About the role
We are recruiting two Learning Disability Nurses to join our Coming Home Project team. The role is open to current NHS Scotland staff only.
Responsibilities
* Support four individuals returning from out‑of‑area placements, working closely with providers to ensure safe, sustainable community care.
* Deliver specialist assessment, intervention and risk management for people with complex needs as part of a skilled MDT.
* Collaborate with CAMHS, Children & Young People’s Services, Education, Social Work and adult LD teams to maintain stable community placements and prevent crisis escalation.
