Role Overview

The single point of access (Community Care Coordination Team) receives referrals from primary care, secondary care, community health and social care professionals, ambulance service, NHS111 and care homes for patients who need clinical interventions to support admission avoidance or discharge home from hospital. The admission avoidance pathways include those which are an alternative to the emergency department to enable patients to be seen at home, or in the right place, at the right time, and prevent unnecessary conveyance or admission to hospital.

This post is a registered clinician position to work within the single point of access to undertake robust clinical triage and be a senior clinical decision maker, identifying the most appropriate clinical pathway for the patient to enable them to be seen in the right place, by the right professional and not be unnecessarily conveyed or admitted to hospital.

This post is maternity leave cover for 18 hours per week till 10th December 2026.


Main duties of the job

The post holder will work in close partnership with the acute sector, social and community service colleagues. They will have in‑depth clinical knowledge and experience, with high understanding of services and pathways to enable appropriate decision‑making and problem solving with referrers to address patient’s clinical needs. This includes the management of clinical risk, coordination of referrals and be accountable for the appropriate utilisation of clinical capacity across the system.

The post holder will liaise at senior level with relevant medical, nursing and therapy staffing to ensure the patient is safely accepted onto the most appropriate clinical pathway, responded to by the most appropriate clinician and receives timely assessment, interventions and diagnostics as required. They may need to liaise with social services, voluntary sector, patients and their families as appropriate.

Person Specification


Education, qualifications & training

* Registered Allied health professional with HCPC or Registered Nurse with RCN or Registered ACP with appropriate registration body
* Educated to diploma/degree level or equivalent experience
* Advanced history taking module undertaking or completing
* Post‑graduate clinical training in an area relevant to the post
* Recognised management or leadership qualification
* Recognised teaching or mentorship qualification equivalent to level 3


Special circumstances

* Valid and current driving licence and car to be able to travel to locations throughout Buckinghamshire


Experience

* Experience of working as advanced clinical practitioner or advanced case manager
* Significant experience of working in a community or inpatient setting
* Experience in leading complex care co‑ordination (including cognitive impairment) and developing highly specialised programmes of care with appropriate advice
* Experience of inter‑agency and partnership working with other professionals from the NHS, Social and Health Care and voluntary organisations


Skills, abilities & knowledge

* Specialist knowledge in managing patients in the community with complex medical needs, long‑term/chronic disease management
* Knowledge of governance and risk strategies and their application in a community setting
* Knowledge of analytical and judgment skills, with the ability to interpret complex situations and make appropriate decisions
* Well‑developed interpersonal skills and ability to build and maintain relationships with other health and social care professionals, patients, carers and the public
* Well‑developed written and verbal communication skills
* Strong IT skills including Microsoft Word, Excel and PowerPoint. Good presentation skills/public speaking.
* Ability to work autonomously without supervision
* The ability to work in emotive and highly charged atmospheres, meet deadlines and work under pressure
* Ability to adapt to the demands of a constantly changing environment
* Ability to challenge and be challenged
* Knowledge of current political drivers, health and social care issues
* Budgetary awareness

£47,810 to £54,710 a year per annum – pro rata for part time.
