Overview
The Reablement Service provides support to individuals who live in their own home. The service is registered with the Care Quality Commission and is based at Wildwood, County Hall Campus. This service operates across the county of Worcestershire and focuses on short-term packages of care to maximise independence. Clients are referred by hospitals to support discharges and by Community Reablement Teams.

Salary: £26,403 – £28,142 per annum
Hours: 37 hours per week

Responsibilities
* Create client records on the rostering system and allocate visits to appropriate care workers
* Update care and visit plans as client needs change
* Maintain workers’ rota patterns, leave requests and training records
* Liaise with care workers daily to ensure all client visits are allocated
* Demonstrate excellent telephone manner and negotiation skills
* Ensure every client receives a visit when due, requiring strong organisational and prioritisation skills
* Maintain high-quality communication and recording, with good problem-solving and the ability to think creatively
* Operate in a fast-paced environment where client needs change frequently and require positive, proactive responses
* Have a good knowledge of Worcestershire
* Work on an 8-week rota within 7:00 to 22:00 daily hours, including 2 weekends; flexibility in working patterns is sometimes needed
Hours & Availability
The Reablement service operates 24 hours a day, every day of the year including bank holidays. The office at Wildwood is manned from 7:00 to 22:00 daily.
