Supporting the Care Manager, you’ll help lead and inspire a team committed to helping people live independently in their own homes. What you’ll be doing • Leading, supporting, and developing a team of Care Professionals • Conducting home visits, care assessments, and quality assurance checks • Introducing Care Professionals to clients to build strong, lasting relationships • Creating and reviewing personalised care plans • Monitoring performance and supporting continuous improvement • Maintaining accurate, compliant care records • Working collaboratively with families, healthcare professionals, and the wider team • Ensuring all care meets CQC standards and company values • Responding to concerns with empathy and professionalism • Participating in the on-call rota and supporting day-to-day operations
