Overview
We are the South Cambridge Crisis Resolution and Home Treatment Team (CRHTT), providing 24/7 care for service users experiencing an acute mental health crisis, to support them to stay in their own home rather than being admitted to hospital. We are based in Cambridgeshire. We are looking for Senior Mental Health Practitioners to join our team. Whether you are a Mental Health Nurse, Occupational Therapist or Social Worker we have an opportunity for you in our team. We are a true MDT and no profession's voice is louder than another's. These roles provide learning and development opportunities, with experience across a wide range of presentations and diagnoses, and development of risk assessment, crisis management, assessment skills and delivery of brief psychological interventions.
You will have the opportunity to work with a wide range of clinicians to support your professional development. We have previously supported staff with non-medical prescribing training, AMHP training and training in psychological therapies.
Main duties
As a Senior Mental Health Practitioner, you will be working within our multidisciplinary team to contribute to dynamic, solution-focused care. The Crisis Team values the diverse perspectives of our colleagues. We work with service users experiencing an acute episode of mental illness, and collaborate to co-develop plans with service users and their families. You will visit service users in their own home to complete assessments, plan and deliver interventions with the MDT, and work with other supporting services where appropriate. An important part of the role is the management of inpatient beds on a rota basis to support bed flow, problem solving and decision-making.
Responsibilities
* Screen referrals to the service and prioritise referrals effectively and safely.
* Provide holistic psychosocial assessment, and plan and implement individual packages of care and treatment.
* Undertake clinical risk assessments and risk management in line with local policies and procedures.
* Provide evidence-based clinical interventions and make autonomous clinical decisions about your practice.
* Develop, plan and implement brief care and treatment interventions for individual patients.
* Ensure timely, effective, and integrated delivery of care.
* Promote a recovery model empowering patients, carers and relatives in decision making and ownership of care plans.
* Act as the patient and relatives advocate; enable management of disability, loss and change.
* Provide information and education to family members, carers and other professionals.
* Liaise effectively with referrers and professionals within CPFT to ensure continuity of care.
* Supervise junior staff as required; act as a role model and clinically expert practitioner.
* Participate as an active MDT member; collaborate with other health care professionals, service users and others in delivering care.
* Champion patient rights (dignity, equality, diversity, choice and respect).
* Be conversant with Government legislation relating to the Mental Health Act, CPA, NHS policies, and Clinical Governance.
* Represent the Crisis Resolution and Home Treatment Team at meetings; ensure good documentation in system records.
* Participate in supervision and training of students.
* Work flexibly over a 37.5 hour week to cover 24/7 service, including on-call if required.
Requirements
Please note for this role you will be required to travel independently around the county, meeting strict time deadlines. You will need to hold a full UK driving licence and have use of a vehicle.
