We are currently looking for a motivated and experienced Occupational Therapist to join the Reablement Team at Bexley Council. This is a full-time, contract role focused on empowering individuals to regain their independence and remain safely at home following illness, injury, or a hospital stay. You’ll work within a multi-disciplinary team to deliver short-term, goal-focused interventions in a fast-paced community setting.
Main Duties for the Occupational Therapist include:
1. Undertaking strengths-based, holistic assessments of adults with a range of physical and cognitive needs.
2. Developing person-centred reablement plans aimed at increasing independence and reducing long-term care needs.
3. Providing clinical input into the use of equipment, minor adaptations, and assistive technology.
4. Supporting service users to achieve functional goals in collaboration with reablement support workers.
5. Working closely with social workers, GPs, therapists, and community health professionals.
6. Reviewing progress and adjusting interventions to meet changing needs.
7. Ensuring all interventions align with the Care Act, Mental Capacity Act, and local reablement policies.
To Be Successful:
8. Experience working as an Occupational Therapist in reablement, intermediate care, or community settings.
9. Knowledge of functional assessment tools and short-term interventions.
10. Ability to set SMART goals and promote independence.
11. HCPC Registration
12. Enhanced DBS/CRB (we can obtain this for you)
13. Eligibility to work in the UK
14. Recognised Occupational Therapy qualification
