Job Summary

The post holder will undertake statutory health assessments with Looked After Children (LAC) for the Local Authority, ensuring that clients/patients have a comprehensive health plan in place which is regularly reviewed in line with statutory timescales or as circumstances change. The post holder will discharge their statutory duty to safeguard and promote the welfare of children in local authority care as part of the Looked After Children Team, and will support other staff within the Health Board in meeting their LAC responsibilities.


Main Duties

* Ensure compliance with the Children Act 2004, the Social Services & Well-Being (Wales) Act 2014 and related guidance, working closely with Cwm Taf Morgannwg staff, multi‑agency partners and other Health Boards.
* Provide specialised guidance and advice on health‑related issues to partner agencies.
* Ensure all identified health needs of LAC placed outside the Cwm Taf Morgannwg area are addressed by working with other health services locally and elsewhere.
* Work flexibly to support the team and assist colleagues from the Safeguarding and Public Protection Team when required.
* Plan and undertake statutory health assessments for LAC, including children and young people with complex needs, within the legal framework and on a timely basis.
* Formulate comprehensive health care plans that reduce the risk of emotional harm and meet identified health needs.
* Identify and coordinate services to meet the complex health needs of LAC.
* Accurately record all data and information in accordance with Health Board policies, maintaining strict confidentiality.
* Interpret and advise on a broad range of health information in various settings, including electronic communication with other professionals.
* Deliver evidence‑based practice to ensure a high standard of service provision.
* Report any child protection concerns in accordance with All Wales Procedures and attend relevant child protection or safeguarding meetings.
* Make decisions in line with organisational and national policies, including the All Wales Child Protection Procedures and the Children Act 2004.
* Work within the framework of local Safeguarding Boards guidelines to promote the safety and wellbeing of children and young people.
* Support colleagues in multi‑agency working to address sexual health, emotional health and risk‑taking behaviours.
* Plan and prioritise own workload, ensuring sufficient time for all role aspects.
* Actively participate in the personal development plan, ensuring regular supervision is accessed.
* Offer clinical support, advice and knowledge to both qualified and student nurses, and provide shadowing experiences.
* Maintain and develop professional skills, ensuring safe and effective performance of duties.
* Maintain professional development, NMC registration and revalidation processes.
* Support implementation of organisational policies related to LAC at the operational level across the Health Board.
* Support delivery of evidence‑based practice in line with legal and statutory regulations concerning LAC.
* Undertake mandatory training and record activity as per Health Board policies.
* Propose changes to working practices or procedures to improve practice.
* Support the development of local LAC policies and procedures.
* Contribute to local and national policy development for LAC.
* Establish clear and effective communication links with children, young people, parents/carers, social care & education staff and other professionals.
* Listen appropriately, communicate sensitive issues to relevant agencies and ensure disclosures are acted upon.
* Utilise effective negotiation and relationship‑building skills with children and young people.
* Respond promptly to feedback or information to implement timely interventions.
* Use electronic communication skills with colleagues and managers.
* Attend reviews and meetings for children and young people, communicating information accurately and sensitively.
* Articulate and provide evidence‑based advice clearly.
* Collaborate with wider networks and other Health Boards in Wales and across the UK.
* Apply analytical and judgmental skills to understand complex and contentious issues such as LAC reviews and child protection conferences.
* Analyse the impact of antenatal factors and adverse childhood experiences on the needs of LAC to support care plans.
* Contribute to multi‑agency partnerships involving LAC.
* Maintain a robust system of health assessments for LAC within a legal framework and timescale.
* Compile and analyse health statistics required by Welsh Government.
* Use IT equipment to deliver daily functions, including data generation and storage.
* Work within information governance arrangements for relevant health information systems (e.g., Myrddin).
* Collate statistics and submit reports as required, maintaining records according to Health Board guidelines.
* Commute across the health community, travelling to sites within and outside the Health Board.
* Maintain security and maintenance of equipment in the working environment, including height meters and scales.
* Manage resources effectively, including environmental, waste and energy policies.
* Participate in audits as required by the Senior Nurse LAC on single or multi‑agency basis.

Qualifications and Experience

* Registered SCPHN/SHN or Health Visitor with current NMC registration – Essential
* Specialised and in‑depth professional knowledge relevant to Looked After Children – Essential
* Evidence of multi‑agency/multi‑disciplinary working – Essential
* Demonstrated significant experience of undertaking health assessments on children and young people, including experience of child development across all ages – Essential
* Knowledge of legislation – Desirable
