We are looking for
band 6 nurse to join our small but friendly team. We provide a 7 day service, 9-5 pm Monday - Friday and 09.15 - 1.00 pm weekends and bank holidays.
The post holder will be based at Bristol Royal Infirmary, working as part of the multidisciplinary team to provide specialist nursing care assessment, education and support to patients who are referred.
The band 6 role in our team will allow you to continue to utilise and develop your acute clinical skills and patient assessment leading to appropriate investigation and treatment as required.You will be involved in caseload management and the assessment of new patient referrals.
This role will enable you to continue to utilise and develop new clinical skills, knowledge and expertise and we will support you with the training you will require including appropriate anticoagulant treatments and patient group directives ( PGDs).
Able to work as part of the multidisciplinary team communicating appropriately and effectively.
Triage of referrals and managing clinic on a daily basis .
Provide care and effective communication with patients and carers in a compassionate manner.
Practice in line with local policy and follow appropriate care pathways.
Able to obtain a patient clinical history from GP's, Emergency Department and other inpatient/outpatient units .
Able to take a full history from patients including clinical presentation, risk factors for VTE, relevant past medical history, family history, social history and medication including allergies.
Able to complete relevant documentation in order to refer patients to other departments, GP and discharge.
Request appropriate diagnostic tests and review imaging results post.
Able to plan appropriate acute or non acute follow up for patients.
Initiate appropriate anticoagulant for patients diagnosed with VTE.
Education of patients and families regarding anticoagulant commenced and appropriate follow up.
