Job Summary:

As a Community Care Assistant, you will provide essential care and support to individuals within their own homes. Working in partnership with individuals and their families, you will help enhance the quality of life for those who require assistance with daily activities due to aging, illness, disability, or other circumstances. Your role will focus on promoting independence, dignity, and well-being, ensuring that each person you support receives compassionate and personalized care.

Key Responsibilities:

* Personal Care: Assist clients with personal hygiene, including bathing, dressing, grooming, and toileting, while respecting privacy and promoting independence.
* Daily Living Support: Help clients with meal preparation, feeding, and hydration as needed, as well as light household tasks like cleaning and laundry.
* Medication Assistance: Support clients in taking prescribed medications at the correct times (with proper training and guidance as per regulatory standards).
* Mobility Assistance: Aid clients with moving around their homes and assist with mobility aids when necessary to prevent falls and promote safety.
* Companionship: Engage in meaningful conversations and activities to alleviate loneliness, support mental well-being, and encourage social engagement.
* Health Monitoring: Observe and report changes in clients' physical or mental health to supervisors or healthcare professionals as appropriate.
* Documentation: Maintain accurate records of daily activities, support provided, and any concerns observed during visits, ensuring clear and professional communication with the care team.
* Coordination and Liaison: Communicate effectively with family members, healthcare providers, and other relevant stakeholders, ensuring a cohesive approach to care.
