StLuke's Hospice are a local charity providing compassionate care to people whose illnesses are no longer curable. We promote dignity in dying and empower people to make the choices they want, from the moment they are diagnosed.

The Patient and Family Coordinator is a new role. Under the guidance and supervision of the Senior Patient and Family Coordinator you will be supporting patients and their families, having an active role in the therapeutic support offered both at the Hospice and within the community.


About the role

* You will have an active role in supporting people who are palliative and affected by cancer and other life-limiting illnesses by providing an holistic assessment to identify the individual needs of patients and families to work with the emotional and psychological support for adults and children.
* A key aspect of the role involves daily and direct communication with patients, relatives, carers and other health and social care professionals.
* You will be expected to attend MDTs, liaising with Hospice community services, our clinical outpatient team, In patient units, and bereavement counselling service
* Communication in this role requires a high degree of empathy, sensitivity, understanding, diplomacy, honesty and integrity.
* Being able to work closely with the patient and family co-ordinator for children and young people to provide holistic family support to both the patient and their families.
* You will be required to have excellent communication and observational skills, clear verbal and non-verbal communication and be an active listener.
* You must approach this role seeing the value in giving someone with a palliative illness full supportive care to enhance quality of life, even at the end of life.

Salary: £25,200 to £30,100 (depending on experience)

Contract: Permanent

Hours: Full-time, 37.5 hours per week

Location: On-site (Basildon and/or Thurrock) - Driving Licence essential


About you - The ideal candidate will have:

* Relevant health or social care experience at AFC Band 3 or equivalent
* Experience of multi-professional working
* Experience in the use of data management
* Experience and good understanding of the complexities of palliative care
* Evidence of good communications skills
* Evidence of good information retrieval skills
* Evidence of accessing information sources
* Understanding of person-centred care
* Understanding of the health & social care environment
* Experience of coaching/teaching patients and carers
