Psalt substance use case worker

We are looking for an organised and highly motivated person centred individual to support the delivery of community drug services within the Swansea region. Kaleidoscope PSALT is a GP with Specialist Interest led service for individuals resident in Swansea aged 18 years +. Opiate Substitution medication is dispensed via a community pharmacy model. Excellent communication between primary care, secondary care and pharmacy colleagues will be required. The Prescription Clark will generate and co-ordinate prescriptions as directed by clinical colleagues. They will ensure that service user wellbeing and safe clinical practice is at the heart of service delivery. They will assist in the overall administrative tasks of the service, and be a critical link in the engagement of service users and community health professionals across the region.


Main duties of the job

Undertake triage assessment and co-ordinate pre-treatment clinical checks.

Facilitate outreach and community engagement to assess, commence and monitor opiate substitute therapy (OST)

Provide advice, information, and guidance on the management of substance use to service users engaged with services.

Agree treatment plans with service users, and review them regularly Undertake drug screening (urine, fingerprint and oral).

Ensure case notes and other statutory records are up to date and kept in accordance with Kaleidoscope policies and procedures / regional information sharing protocols

Deliver harm reduction interventions, including HCV testing and naloxone training/administration where appropriate and in partnership with Local Health Board and Public Health Wales colleagues.

Liaise with community pharmacies to ensure prescriptions are in place and monitored appropriately. Participate in clinical audits and service reviews

Support referrals/ liaison with Community Mental Health Teams (CMHTs) and other community substance use treatment providers on entering and leaving Kaleidoscope services.

Actively encourage service user involvement and feedback and ensure views are considered in informing the development of the service.

Liaise with primary care and sexual health services.

Deliver targeted recovery orientated psychosocial support interventions.

Be responsible for undertaking and recording patient observations eg. BAC, BP, Weight, Temp, O2 sat. etc

Professional Development

Maintain and develop personal expertise and knowledge by reading, networking and further training to develop professional competence and to contribute to the development of competence within the team.

Help with the induction and orientation of new staff, volunteers, and students.

Ensure professional development by seeking regular supervision and annual appraisals.

Effectively manage own time and prioritise workload, maintaining own administration and accurate personal records regarding travel, diary appointments, TOIL, and expenses incurred.

Be accountable and responsible for own professional practice, operating in line with professional code of conduct and within organisational policy and procedures.

Clinical Governance

You will deliver services in line with Welsh Government Treatment Frameworks, NICE guidance (National Institute for Health and Clinical Excellence), and other relevant professional standards and legislation.

Kaleidoscope staff are expected to follow Operational Policies and Standard Operating Procedures (SOPs), ensuring that the care and treatment provided is safe, undertaken to a high standard and informed by up-to-date, evidence-based practice.

* Access to own car and insurance that includes business use cover


Qualifications

* NVQ Substance Misuse / NVQ Health & Social Care


Experience

* Relevant training / experience in alcohol screening / brief advice / drug treatment services
* Experience of using electronic case management system (e.g. PARIS / WICCIS / PalBase / Clinical
* Workstation).
* Ability to manage own workload remaining calm under pressure.
* Ability to manage professional boundaries, deal with sensitive information and maintain strict
* confidentiality both in and out of the workplace.
* Able to use own initiative with direction
* Experience of delivering outreach and community nursing models.
* Training in Blood Borne Virus screening and anaphylaxis
* Experience of developing and maintaining successful partnerships with GPs, treatment agencies and
* other healthcare providers.
