Job Summary

The purpose of this post is to provide a neurodisabilty paediatric consultant to further develop and lead within a subspecialty interest. The post holder will be expected to help provide clinical services for Gloucestershire, together with appropriate teaching and supervision of junior doctors and medical students. They will be expected to work closely with primary and tertiary care, as well as non-health agencies, to provide a seamless service.

As a senior employee of the Trust the post-holder will work in close co-operation with, and support other clinical, medical professional and managerial colleagues in providing high quality healthcare to the Trust's patients.

Main duties of the job

Integral to the responsibilities of the post are the following requirements:

To ensure the provision of a first-class clinical service, including safeguarding.
To provide effective leadership to all staff engaged in the specialty
To sustain and develop teaching and research wherever appropriate
To undertake all work in accordance with the Trust's procedures and operating policies
To conduct clinical practice in accordance with contractual requirements and within the parameters of the Trust's and Division's services plans
To maintain the confidence of business plans and development strategies formulated for the specialty, the Women and Children's Division or the Trust

About Us

The medical team consists of consultants and specialty doctors, working alongside and supported by an established team of specialist nurses (epilepsy/neurodisability). We also have paediatric dietetics, home enteral feeding team and specialist respiratory nursing to support the care of our patients with complex chronic conditions. There is close collaboration with an active community children's nursing team and therapy teams employed by Gloucestershire Care Services NHS Trust. Many of our patients gain access to excellent Children's Hospices in our reach: ACORNs Worcester and Helen House Oxford, and the James Hopkins Trust for our young children with respite needs. There is a current service review of palliative care provision. Children with learning disability have access to specialist learning disability nursing support (delivered by Gloucestershire Health and Care).

Over recent years, neurodisability services in Gloucestershire have evolved to include and develop close working relationships with the following:

Clinics centrally at CGH and GRH, andoutreach clinics covering the breadth of the county, providing clinic opportunities close to home for most families. Close collaborative working with special needs schools.
Neonatal follow up program for at risk neonates.
Local genetic services, including specialist neuro-genetics and genomics.
Specialist MDT feeding clinic.
Paediatric epilepsy service.

Job responsibilities

SPECIFIC RESPONSIBILITIES OF THIS POST

The postholder will be expected to develop and lead an area of specialist interest to support countywide services. Excellence of care will be expected and the postholder will take full part in the risk management and clinical governance process for the department.

Clinical

To provide expertise in the assessment and investigation of young children presenting with a range of developmental/neurological/genetic problems; to provide continuity for those children who require ongoing clinical care due to their developmental condition. (for example children with neurodegenerative conditions, cerebral palsy, metabolic conditions, neuro-cutaneous and other syndromes).
To provide expert assessment of children presenting with social communication /neurodevelopmental disorders and provide medical management of ADHD (as part of CAAAS MDT service).
To provide clinical continuity and contribute to care when their patients require hospital attendance or admission to the ward.
To facilitate transition of clinical care to adult services.
Participation in a child protection on call rota, covering normal working hours Monday-Friday. To attend strategy discussions to provide a paediatric opinion on children presenting with safeguarding concerns. Carry out child protection medical assessments where appropriate. Participation in strategy discussions for CSA (CSA examinations currently carried out by Bristol SARC).
To contribute to a countywide consultant led service, and promote an increase in service provision and capacity within the available resources.
To share responsibility for the delivery of high-quality care to the patients presenting to the paediatric service.
To support the clinical decision making of junior medical and nursing staff.

Teaching/Training (as agreed with Specialty Director)

To provide conditions for improved training opportunities in line with national and local recommendations arising out of Modernising Medical Careers and related requirements.
To take a full role in undergraduate and postgraduate teaching, including clinical meetings and tutorials. Medical students are currently taken from several universities, but the Clinical Academy based on the University of Bristol is expected to provide the majority of undergraduates in future. #Where agreed, to act as Educational Supervisor to junior and middle grade medical staff, being responsible for their appraisal and personal development planning.
The Trust subscribes to a wide range of journals and there are good medical libraries at both Gloucester and Cheltenham with full literature search facilities. The department is part of a Trust wide intranet system and is well served for literature search and other internet functions.

Quality Improvement, Audit and Clinical Governance (as agreed with Specialty Director)

To actively participate in clinical governance and risk management processes.
To promote evidence-based practice; to respond appropriately to NICE guidance; to change own and others clinical practice in response to new clinical developments.
To undertake and supervise relevant clinical audit topics.
To remain up to date and engaged in continuing professional development (CPD) as per Royal College of Paediatrics and Child Health and General Medical Council guidelines.
To respond appropriately to complaints and critical incidents.
To undertake all work in accordance with Gloucestershire Hospitals NHS Foundation Trust procedures and operating policies.
To attend accredited conferences and meetings to update personal level of clinical practice, teaching and management skills in line with Continuing Medical Education requirements.
To participate in an annual Job Planning Review process.
To participate in the Trusts annual Appraisal process.
To adhere to the Trusts Adverse Clinical Incident Policy.

Person Specification

Essential

Qualifications and Training

Full GMC registration
MBBS or equivalent
Entry on Specialist Register (or entry expected within 6 months)

Desirable

Level 3 (multiagency) child safeguarding training
A period of training in a specialist neurodisability and/or neurology unit (or equivalent)
Experience in multidisciplinary team working; ability to offer expert clinical opinion on disability, child protection and on wider aspects of community paediatric care
Qualification in healthcare management or leadership

Knowledge and Skills

Consistent with independent consultant practice in general paediatrics with a neurodisability / community paediatric subspecialty interest.
Competent to assess and lead on the safeguarding aspects of children presenting following possible child abuse.
Evidence of participation in Audit, Clinical Governance and Quality Improvement project
Ability to manage risk

Desirable

Competent to take charge of the day to day and longer-term clinical management of children with chronic medical conditions, in particular neurodisability disorders.
