Omni-Care was established in 1999 as a small family operation providing home care support to people who are elderly or have a disability, and those who care for them. From these humble beginnings we have grown to service all areas of metropolitan Melbourne and currently have a workplace of approximately 350 support workers and around 60 office staff. The Role

We are seeking a Care Coordination Assistant who will be responsible for assisting the Care Coordination team in achieving and maintaining optimum customer service levels. This role involves coordination of support workers to attend clients and other administrative tasks. The role reports to a Care Coordination Team Leader and is a stepping stone to the Care Coordinator role and beyond.

The position involves:

Contribute to Omni-Care's positioning as a premium provider of attendant care services by maintaining excellent customer service standards and ensure clients rights are respected in every aspect of the role.

Monitor workspaces and follow up with support workers to ensure that there are no missed shifts and other compliance requirements are met.

Complete client confirmation calls where required and/or confirmation documentation.

Take overflow support worker calls for the team you are connected to e.g. support worker providing feedback and entering appropriate notes into the GoldCare system and forwarding the notes to the Care Coordinator responsible.

Record compliments and complaints and when experienced, assist with other Care Coordination tasks.

Complete data management tasks and other projects as required by the Team Leader or Care Coordination Manager.

About You

Great attention to detail.

A positive personality and a can-do attitude.

Experience in aged care or disability or a strong administrative background will be well regarded.
