An exciting opportunity has arisen within the Rotherham Foundation Trust for a Clinical Nurse Specialists Band 6 within the already established Tissue Viability nursing team. As a Clinical Nurse Specialist you will act as an advocate for patient care, providing seamless and timely management for patients with compromised skin integrity.
This fast paced and varied role requires an individual who shows a methodical approach to patient assessment who will work alongside the wider team to support patients, providing an individualised plan of care, delivering a high-quality service and ensure the service is accessible and available to give advice and clinical expertise to both patients and other health care professionals.
We are looking for an individual to work alongside the existing team, who is caring and shows enthusiasm. The ideal candidate will be able to work effectively with both patients and the wider MDT whilst working cohesively within the Clinical Nurse Specialist Team. This unique opportunity requires you to be committed, organised and an excellent communicator both verbal and written.
You must be able to work both independently and as part of a team. We are keen to invite to interview Nurses who encompass the above and meet the essential criteria in the person specification.
Main duties of the job
The main duties of the role involve assessing, planning, implementing and evaluating specialist nursing care programmes, always considering the patients physical, psychological and psycho-social needs.
The postholder will collaborate with colleagues on care processes, integrating current evidence and best practice while reviewing interventions for effectiveness and timeliness.
Responsibilities include making autonomous clinical decisions, managing a personal caseload, and acting as an advocate for patients under the tissue viability service.
The role demands close liaison with multidisciplinary teams, participation in case discussions, and timely referrals to other specialists when required.
The postholder must ensure patients and families are fully informed, support them in making decisions, and set realistic expectations regarding outcomes and side effects.
Additionally, the duties involve supporting patient and carer wellbeing, maintaining accurate clinical records, contributing to MDT meetings, and using specialist knowledge to upskill colleagues in caring for patients with compromised skin integrity, all while upholding privacy, dignity, and professional standards.
