Role Overview
The Womens CSU Quality and Safety team leader – PMRT Lead will work under the direction and guidance of the Womens CSU Governance and Quality Lead. They will be responsible for the line management of Band 6 Nurses/Midwives and the administrative staff based in the Quality and Safety team, and for supporting the patient safety and quality improvement agenda within the Women's CSU.
The key purpose of the role is to support the Womens CSU Governance and Quality lead to deliver the patient safety and quality improvement agenda, to contribute to the development of governance and quality improvement processes, and to monitor and review incidents and investigations. The post holder will identify opportunities for shared learning of highly complex, highly sensitive or contentious information and will disseminate learning to the wider multidisciplinary team. They will also provide daily oversight and monitoring of all CSU internal and external reporting processes linked to adverse incidents, morbidity and mortality.
Job Responsibilities
* Lead, manage and coordinate the full PMRT process for all applicable perinatal deaths.
* Ensure reviews are completed within national timescales and to required standards.
* Facilitate multidisciplinary review meetings, ensuring appropriate attendance and high‑quality discussion.
* Maintain accurate, timely and confidential records of all reviews.
* Maintain an accurate and accessible PMRT database; generate reports for local, regional and national use.
* Provide expert advice on PMRT methodology, standards and best practice.
* Ensure compliance with the Maternity Incentive Scheme and Saving Babies Lives programmes.
* Lead, manage and coordinate the full Mortality and Morbidity process for all applicable gynaecology deaths.
* Ensure parents are offered the opportunity to contribute to the review and that their perspectives are incorporated sensitively and effectively.
* Ensure all families receive compassionate communication and are fully informed of the review process.
* Work closely with Bereavement Midwives and Patient Experience teams to ensure family feedback drives service improvement.
* Represent the parent voice within the PMRT process.
* Identify opportunities for shared learning and disseminate findings to the multidisciplinary team.
* Oversight and monitoring of internal and external reporting processes linked to adverse incidents, morbidity and mortality.
Qualifications
* Registered Midwife or Nurse.
* Degree in Midwifery or Nursing.
* Evidence of continuing professional education and development.
* Qualification or working experience in root cause analysis.
* Formal leadership training.
Experience
* Experience in clinical audit.
* Knowledge and experience of the national patient safety agenda and NHS England and CQC priorities.
* Demonstrable experience of managing a team.
* Knowledge and understanding of leadership theory and ability to align with practice.
* Understanding of the establishment setting and budgets.
* Knowledge and understanding of all key HR policies and procedures.
