Schedule: 12-hour variable shifts
SUMMARY:
The licensed practical nurse performs tasks and responsibilities within the framework of case finding, health teaching, health counseling, and provision of supportive and restorative care under the direction of a licensed health care provider.
RESPONSIBILITIES:
* Collects patient data related to patient and family needs
* Performs tasks related to the comprehensive plan of care involving the patient and the family.
* Performs and documents direct patient and family centered care according to nursing standards of practice.
* Responds to patient conditions to achieve positive patient outcomes.
* Performs tasks related to discharge planning to ensure safe and appropriate transition of care.
* Supervise unlicensed team members in the delivery of care.
* Provides world class customer service to patients and families.
REQUIRED QUALIFICATIONS:
* Graduate of an accredited practical nursing program
* Current, unrestricted New York State licensed practical nurse license.
PREFERRED QUALIFICATIONS:
* One year of post-licensure practice as a Licensed Practical Nurse preferred.
PHYSICAL REQUIREMENTS:
M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.
