Overview
Social Worker - Birmingham City Council, Hospital Sector Discharge to Assess Teams. 36.5 hours per week. Permanent, Grade 4, with a salary of £34,434-£41,771. Golden Hello payment of £1,000 plus £5,000 per annum (see details and conditions).
An exciting opportunity has arisen to join the Hospital Sector Discharge to Assess Teams based within various locations. Successful candidates will work across busy multi-disciplinary teams to facilitate social work practices.
Responsibilities
* Deliver person-centred practices where citizens are enabled to exercise choice, control, independence, and build resilience with a focus on prevention and early intervention.
* Provide guidance, signposting and assessments in accordance with the 3 Conversations model and the Care Act 2014.
* Carry out social work functions within the legislative framework, using relevant legislation to complete conversations, provide support and guidance, and produce reports under required timeframes.
* Work as part of the Discharge to Assess Social Work Teams to deliver on the above in collaboration with a multi-disciplinary team and other stakeholders.
* Demonstrate the ability to work under pressure and maintain professional standards in busy settings.
Qualifications
* Eligible to be registered with Social Work England.
* Ability to work effectively within a multi-disciplinary team and with stakeholders.
* Interest in and skills to work within the Discharge to Assess Multi-Disciplinary Team.
* Ability to produce reports and communicate clearly; proficient interpersonal and communication skills.
* Eligible to work in the UK and meet the English language requirements, as required by the Immigration Act 2016.
Requirements and Conditions
