One young person's successful transition to the young adult provision (2026)

Thatcham
Prior's Court Foundation
Posted: 9 March
Offer description

Once the young people at Prior's Court reach the age of 19, they often either go onto a different further adult provision, or are transitioned up to Prior's Court's. Combining with a person-centred approach, the Young Adult Provision aims to provide a transitional step towards a more inclusive life - developing independent living skills, communication, social skills, leisure and vocational opportunities and enabling autistic young adults with complex needs to lead a fulfilling and purposeful life.

Below is the story of one young adult moving up to the Young Adult Provision and the collaborative work of different teams across Prior's Court ensured this transition was smooth and the young adult was settled.
