Hours: 15 hours a week
The post holder will be responsible for assessing, screening, treating and advising and educating patients, working as part of the primary health care team. They will deliver a wide range of defined patient care, including basic treatment room care, chronic disease management and contributing to health care promotions.
Responsibilities
* Patient consultations
* Performing clinical procedures such as phlebotomy, administering injections, wound care
* Chronic disease management: working with patients to manage conditions such as asthma and diabetes through regular reviews and education
* Health promotion: promoting healthy lifestyles, providing family planning advice, immunisations, etc.
* Collaborating with GPs to provide high quality care for patients

Job Description
Health education and advice to patients on topics such as healthy living and disease prevention. Chronic disease management (asthma, diabetes, COPD, etc.). Cervical cytology, Baby and childhood immunisations, Wound care, ECG, Spirometry, Immunisations, Travel health, Anticoagulation management, Contraception checks, Phlebotomy, Smoking cessation, Ordering of vaccines, Supervising the Phlebotomist, Organising the Seasonal Flu campaign. Supporting the practice in contractually meeting clinical targets from NHS Wales. Ensuring compliance with relevant legislation, regulations and quality standards. Maintaining accurate patient records and ensuring smooth flow of information within the practice. Adhering to the NMC Code of Professional Conduct and practice protocols.
Qualifications
Essential: Registered Nurse with valid registration with the Nursing and Midwifery Council
Desirable: Current CPR training; Level 2 child protection
Experience
Essential: Previous experience of working in a similar job role within Primary Care; Knowledge of needs of patients with long-term conditions; Awareness of accountability of own and others roles in a nurse led service; Knowledge of health promotion strategies; Awareness of clinical governance issues in primary care; Knowledge of patient group directions and associated policy; Change management skills and ability to support patients to change lifestyle; Excellent communication and organizational skills; Empathy and sensitivity in patient interactions; Willingness to undertake further training; Proficiency in IT systems (e.g., Microsoft Office, clinical systems); Strong interpersonal skills; Motivated, adaptable, flexible, and able to work under pressure; Good understanding of the NHS and primary care landscape; Ability to work effectively with individuals and teams
Desirable: Knowledge of public health issues in the area; Awareness of local and national health policies; Awareness of issues within the wider health economy; Compression bandaging; Doppler studies; Knowledge of EMIS Web Clinical system; Knowledge of GMS Contract
