Role Overview
The successful candidate will work as part of a multidisciplinary therapy team, assisting with the delivery of therapy under the supervision of Occupational Therapists, Physiotherapists, and Speech and Language Therapists. Responsibilities include implementing therapy plans in hospital or community settings, such as exercise interventions, occupational input, speech and language input, environmental assessments, and equipment prescription within competence.
The post holder will communicate effectively with Stroke Therapy Services and the multidisciplinary team to support administrative duties and planning for safe discharges. This is a Trust-wide position, and the candidate may work across Stockton and Hartlepool localities.

Key Responsibilities
* Support and rehabilitate patients to enable safe return home or placement in safe environments with ongoing care.
* Implement delegated therapy plans confidently, including communication, eating and drinking, mobility, and functional interventions.
* Perform administrative tasks and home visits within competency, providing equipment and resources as needed.
* Make minor modifications to therapy plans and fit or maintain equipment and communication aids.
* Monitor and manage therapy stock and resources.
* Coordinate with partner agencies for safe discharge planning.
* Accurately record interventions and communications, ensuring informed consent and understanding.
* Comply with Trust policies on risk management and infection control.
* Participate in team development, supervision, and appraisal processes.
* Contribute to clinical audits and ensure timely submission of performance data.
* Perform duties requiring moderate physical effort and work extended hours or as part of a seven-day service when needed.
* Undertake other duties as deemed appropriate by the Clinical Lead or Senior Professional.
